Provide requester information

The Basic tab displays the program and information about your request. While you may change the program, the requester information is fixed and cannot be edited. Provide the remaining details that are marked by an asterisk ( * ) such as the contact person.

For requests that will need more than one sample, you may indicate the number of samples per item in the textbox provided.

Once done, click Next.

Great job! You may now proceed with adding the service information.
